RN Patient Navigator, Bariatric Clinical Unit - Main Campus - ( 260004J1 ) Description A Brief Overview RN Patient Navigators provide high-touch customer service to UH customers, building strong relationships while helping them navigate the healthcare system. What You Will Do: - Coordination of care and access of specialty and general medicine care and screening. - Community resource referral, patient education, and navigation/entry point in to UH. - Actively participates in the process for operational improvement and quality improvement. - Involved in learning and self-improvement. - Provides leadership through communication, education and management. - Provides advocacy for the patient and family. - Tracks interventions and outcomes of patients and families who access UH. - Initiates and maintains positive relationships with co-workers and physicians Additional Responsibilities: - Demonstrates commitment to the mission, vision and values of UH. - Performs other duties as assigned. - Complies with all policies and standards. - For specific duties and responsibilities, refer to documentation provided by the department during orientation. - Must abide by all requirements to safely a...
