Description The Discharge Planner is responsible for identifying patient needs and priorities for care through assessment, planning, intervention, and evaluation. The Discharge Planner coordinates care between professional disciplines through documentation and oral communication as needed. The Discharge Planner will provide psychosocial support to patients and families so they can cope with chronic, acute, or terminal illness. Discharge Planner may advice family caregivers, counsel patients, and help plan for discharge needs. Requirements: - License to practice nursing RN or LPN in the state of Idaho or qualified personnel. - Current BLS certification. - ACLS and PALS recommended but not required. - Ability and willingness to demonstrate and maintain competency as required for job title and the unit/area(s) of assignment. - Excellent communication skills to include written and verbal comprehension and expression. - Ability and willingness to work with patients’ growth and development needs particularly related to the age of patients in the unit/area(s)of assignment. - Ability to manage a chaotic work environment related to changing patient needs, including working with patients wit...
