Responsibilities: · Provide a nursing evaluation visit to new admissions. · Regularly re-evaluate patient needs. · Consult with the patient’s practitioner. · Complete bedside documentation using the EMR. · Coordinate and manage services offered by team members. · Help ensure the final chapter of your patient’s life is one of quality, compassion, and dignity.
