Description JOB SUMMARY: Within the context of a client and family centred care model, the vision and values of the organization, the Home Health Dietitian works independently and in conjunction with the interdisciplinary team in client’s homes or in the ambulatory care setting to advocate, develop, implement, and evaluate nutrition care plans regarding oral and/or enteral nutrition support regime(s). Develops and supports food security education and guidance to clients. Acts as a consultant to staff, community agencies and stakeholders to provide consistent, reliable, evidence based nutrition related information. Acts as a resource person to combat malnutrition in community settings by implementing, evaluating and sustaining community based intervention programs. QUALIFICATIONS: Education & Experience Current full registration as a Dietitian with the College of Health and Care Professionals of BC (CHCPBC). Two years recent related experience or an equivalent combination of training, education and experience. Restricted Activity A & C is required to practice.Valid BC Driver’s License.
