POSITION SUMMARY: Coordinate and provide case management services, ensure quality of care to the consumer population and assist each individual in maintaining his/her independent living environment. ESSENTIAL ACCOUNTABILITIES: 1. Responsible for the operation of the Supported Housing program and assigned caseload. Drive self and/or drive agency vehicle throughout boroughs to visit apartments routinely, to assess, observe and evaluate physical plan and residents functioning abilities and needs. 2.
