OVERVIEW: The Lead Care Manager (LCM) supports children, youth, and families in various community settings in collaboration with the treatment team. The LCM's role is to coordinate all aspects of Enhanced Care Management (ECM) and any Community Support (CS). DUTIES: 1. (90%) Direct Services Provide support services to children and families via methods consistent with the agency’s treatment philosophy and approach.
