Reliance Hospice and Palliative Care is looking for a Full-Time Licensed Vocational Nurse (LVN) to join our team! You will be supporting our locations around San Fernando Valley Area. Regular Hours: Monday to Friday, 8am to 5pm Job Description: - Providing nursing care to hospice patients in private homes, skilled nursing facilities, board and cares, and assisted living facilities under the supervision of an RN. - Assist in obtaining the full assessment, following accurate orders, diagnosis and projection of visits for all disciplines necessary to support each patient. - Perform and document nursing interventions including patient education according to the Plan of Care. - Understands and is competent in workflows, problem charting, interventions and accuracy of input by review of POC at each visit. - Demonstrates knowledge and competency in performing baseline nursing skills. - Identifies and determines priority of the patient's needs/problems and reports any variance to the RN. - Administers all medications and treatments. - Communicates effectively with the RN, patients, families and physicians. - Accurately document patient status, patient needs/problems, nursing interventions/...
