Park Nicollet is looking to hire a Registered Nurse to join our Home Care team! Position Summary: The Home Care registered nurse assesses and evaluates patient care needs, provides skilled nursing care and teaching to patients and families in their home or community - based upon community standards of practice. The nurse is responsible to improve or maintain patient’s health status, provide terminal illness care, and to prepare patients and their families for self-directed and/or assisted care in their community setting. The nurse coordinates medical, social, and financial resources to develop a comprehensive plan of care. The nurse supports the following community care processes: admission, discharge, billing, referrals, bereavement, and care delivery. Work Schedule: M-F 8a-4:30p, occasional evenings and on-call. Every 6-7th weekend. 1-2 holidays/year.
