Community Home Care - Intake Coordinator

The Community Intake Coordinator is accountable for receiving, reviewing, reconciling, and processing all incoming private pay referrals, including receiving referral via fax, phone, or email; determining if referral can be accepted based on resource for payment, service area, and scope of nursing and/or caregiving needs; review referrals for necessary components and determine missing components; reconcile missing referral information through communication and coordination with TCUs, Primary MD clinics, hospitals, etc; process and oversee the processing of referrals into EMR; coordinate referral flow and capacity with department leadership team; identify additional needs and coordinate to meet client needs. This position is also responsible for assisting with client record management including faxing and tracking physician orders and maintaining client information in the EMR. Position: Intake Coordinator for Community Home Care Hourly: $25.00 - $29.00 hourly, depending on experience Service Area: Twin Cities Metro area. Plus, mileage reimbursement of 72.5 cents per mile.
