Department: Community Care Network Shift: Primarily Days (United States of America) Employee Type: Regular Per Diem Type (if applicable): Minimum Pay Range: $71.73 - $86.07 Job Description: POSITION SUMMARY The Manager, Care Coordination is responsible for the leadership, development, and oversight of care coordination services in outpatient and community-based settings with a team comprised of Health Navigators, RNs, and Medical Social Workers. This role focuses on advancing population health outcomes through proactive outreach, care management, and interdisciplinary collaboration. The Manager ensures high-quality, patient-centered care while optimizing resource utilization, improving access, and reducing health disparities across defined patient populations.
