Job Overview: The Activities Coordinator is a member of the PACE Interdisciplinary team (IDT) and is responsible for actively participating in all care coordination activities, including Assessments and Care Planning. This role involves designing, implementing, and overseeing a variety of engaging activities tailored to the physical, cognitive, social, spiritual, and social well-being of the PACE participants.
