JOB CATEGORY: CARE COORDINATION / CASE MANAGEMENT / SOCIAL WORK / REGISTERED NURSE POSITION SUMMARY: Under licensed supervision, the Care Coordinator II – Delegated works closely with the patient/family and the Interdisciplinary Care Team to proactively assess, plan, implement, monitor and evaluate options and services to effect an appropriate, individualized plan for the patient across the continuum of care. The process addresses the patient’s multiple environments of care and healthcare system needs. The position requires a focus on customer service skills, knowledge of goal setting, and measuring outcomes to effectively ensure the patient has the most optimal outcomes provided in the most cost-effective manner. Adheres to procedures and timelines as specified in the Managed Care Organization (MCO) contract and PMS procedures.
