Serves as a key resource for individuals with diabetes, those at risk for diabetes, and healthcare professionals by providing education, tools, and guidance that support improved health outcomes. Plans and implements community outreach and public awareness initiatives that promote diabetes prevention and control. Activities may include coordinating health fairs, community events, media outreach, and presentations to civic groups while distributing educational materials aligned with the Kentucky State Diabetes Program. Facilitates evidence-based education programs that encourage healthy lifestyle changes among individuals and families affected by diabetes or prediabetes.
