Job Description: ESSENTIAL JOB FUNCTIONS - Make appointments and/or referrals for PCP or Medical Specialty. - Maintain the Integrated Care Coordinators schedule including appointment reminders, scheduling appointments and rescheduling following a cancellation or no show. - Prepares and mails caring contacts to clients determined to be at a risk of suicide as directed by Integrated Care Coordinator. - Assists clients in navigating the healthcare environment to ensure appropriate access to care. - Assists with coordination/access to services that address social determinants of healthcare outcomes: - Assist with options for transportation to healthcare appointments - Assist client in obtaining housing resources - Assists with access to other community resources e.g., public assistance and financial benefits - Develops knowledge base of other healthcare providers in the service area and collaborate, as needed - Documents tracking for appointments, medication, and other healthcare needs for assigned clients, as needed EDUCATION: - High School diploma or GED - Preferred, but not required training as a Medical Assistant EXPERIENCE: - One year of experience working in a hospital, physician...
