Overview Helio Health's In Community Program is designed to help individuals and families, who have encountered certain obstacles develop the resources and supports to live a satisfying, naturally independent life. The Assisted Outpatient Treatment (AOT) Health Home Plus Care Coordinator works with individuals recovering from serious mental health disorders who are returning to the community from the NYS Correctional system or the NYS Psychiatric Hospital system or are a participant in the Assisted Outpatient Treatment (AOT) program. Pay Rate: $25.00 per hour: In addition to Helio Health’s comprehensive benefits package, this position is eligible for a $1,500 Sign-On Incentive!
