Population Health Specialist Overview: The Population Health Specialist- Care Transitions Coordinator works within the Population Health Department to assist with the day-to-day operations of managing care transitions including work to coordinate follow up care after inpatient admissions, ED visits and care management program enrollment. They will also assist with generating claims for home health oversight and care management activities This position reports directly to the Senior Manager of Population Health. Essential Functions and Duties: Demonstrate effective teamwork, prioritization, communication and organizational skills Maintain accurate spreadsheets and data reports for all pertinent programs Assists in the development of workflow processes for all pertinent programs Assists with Transitional Care Management activities as needed, including patient outreach for screening events Assist with enrollment and claims generation for care management programs - Other duties as assigned Knowledge/Skills/Abilities: Excellent verbal communication and active listening skills, conflict resolution, integrity, empathy, compassion, patience Ability to multi-task and independently manage di...
