System Navigator 1 FTE: Thunder Bay Site One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services. We are currently seeking a 1 FTE System Navigator- Team Care as a valuable member of the Community Health Centre team, will provide support for clients with complex and multiple needs in the communities that we serve. The position has a competitive salary, the Health Care of Ontario Pension Plan and an excellent group health benefits package. The salary range for this position is from $53,897 to $64,932 per year. We currently do not use AI to screen resumes. Qualifications: - Honours Bachelor degree in Social Work (HBSW) or Registered Practical Nurse diploma (RPN). - Minimum one-year work experience. - ASIST Training - Ability to effectively engage clients in the assessment and planning with regards to their needs. - Demonstrated ability to facilitate and advocate for services to support clients in their care. - Ability to work both independently and collaboratively with an interdisciplinary team. -...
