Do you want to help to make every person’s discharge from the acute hospital setting and the community go as well as possible, to the right place at the right time? We are looking to recruit a Transfer of Care Facilitator to develop and implement trusted assessment processes between hospital and community teams and Care Home providers to improve the process for everyone and reduce delays, enabling proactive discharges to care settings, where this is required.
