The LPN/LVN provides professional nursing care to patients within an assigned unit, clinic or region. You will work on-site at the Conviva De Leon Care Center. You will report directly to the Area Operations Director. Job Functions: Use your skills to make an impact - The LPN/LVN assesses patient health problems and needs. - You will take vital signs, administer medication, develop, and implement nursing care plans, and maintain medical records. - You will advise patients on health maintenance and disease prevention or provide case management. - You will need to understand own work area professional concepts/standards, regulations, strategies, and operating standards. Required Qualifications: - You will have a current licensure as an LPN or LVN - This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program.
