Become a part of our caring community and help us put health first As a Patient Care Coordinator, you will: Use your skills to make an impact - Follow up on Face-to-face requirements, educating physicians on CMS guidelines for Home Health in person, or virtually by email or phone. - Visit different VA offices in NKY/Cincinnati area to obtain orders/signatures and follow up on VA patients. - Attend Tuesday business development meetings, bringing relevant data, reports, and information about changes within accounts and referral sources. - Report to a Market Executive of Sales - Work an 8 hour shift every Tuesday/Thursday or Tuesday/Friday Required Experience/Skills: - RN/LPN/LVN license. - Three years of nursing experience is preferred - Must have knowledge of governmental regulations, Medicare eligibility requirements, comprehensive understanding of potential care plan needs for the patient and coordination of necessary resources. - Excellent customer service, account development capabilities, organization, time management, problem-solving, communication and selling skills. - We require travel within an assigned territory.
