Case Manager, Clinical Stabilization Services (CSS) - Hours : 7am - 3:30pm from Friday to Monday. You must be able to work this schedule every week - Pay : $29/hr - $38/hr dependent on related experience, education and licensure Summary/Objective: The Case Manager is a key member of the Clinical Stabilization Services (CSS) treatment team and provides care coordination and discharge planning for individuals receiving residential substance use treatment. This role works closely with clinicians, recovery specialists, medical providers, and community partners to help clients access services, address barriers, and successfully transition back into the community. Essential Functions: - Meet regularly with clients to provide care coordination and aftercare planning. - Conduct assessments to identify client needs, strengths, and barriers to treatment goals. - Develop care plans in collaboration with clients and the treatment team. - Coordinate referrals and linkages to services including housing, employment, healthcare, and financial supports. - Coordinate care with outside providers such as courts, primary care providers, and behavioral health specialists. - Facilitate psychoeducational...
