Job Title: Licensed Practical Nurse (LPN), ElderONE (Part-Time, Varied) Department: Homecare: Location: ElderONE - North Park Pace Center 355 N Park Dr, Rochester, NY 14609 Hours Per Week : 20: Schedule: 7:00am-3:30pm, 3:00pm-11:30pm: SUMMARY: The Licensed Practical Nurse is responsible for providing direct patient care, usually under the direction of a Registered Nurse, in accordance with established plans. This position has a designated home base facility and may require occasional travel to additional locations. Typically, travel is limited to one to two locations per day, with up to four sites in Rochester, NY and one in Newark, NY. Mileage reimbursement is provided. ATTRIBUTES: Strong communication, customer service, and interpersonal skills highly desired.1 year of experience working with a frail or elderly population preferred.
