POSITION SUMMARY: The Accountable Care Organization (ACO) Case Management Nurse is responsible for reviewing patient medical records to identify preventative care measures and screenings, such as mammograms, colonoscopies, immunizations, medication adherence, and more. Once these needs are identified, the ACO nurse coordinates and schedules the appropriate preventive services and wellness visits. This role requires flexibility, as the nurse may be called upon to assist in other areas of the clinic as needed. Duties must be performed in a manner consistent with the Mission and Values of Hattiesburg Clinic.
