Community Health Worker 1 FTE: Thunder Bay Site One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services. We are currently seeking a 1 Full Time Community Health Worker. Community Health Workers, as valuable members of the Community Health Centre team will provide supports for clients to improve health and well-being, reduce social isolation through the delivery of programs and connect clients with long-term services in the community. The Community Health Worker works closely with the inter-professional care team and community partners to support clients with their needs and goals related to the determinants of health. The position has a competitive salary, the Health Care of Ontario Pension Plan and an excellent group health benefit package. The salary range for this position is from $53,897 to $64,932 per year.
