HIV Psychosocial Support Advocate - Pay: $26.42 per hour - Schedule: Monday – Friday, 9:00am – 5:00pm Summary/Objective: The Psychosocial Support Advocate provides psychosocial support services to individuals living with HIV/AIDS. This role develops ongoing relationships with clients and supports engagement in medical care and social services. Responsibilities include appointment reminders, accompaniment to appointments, care coordination, and facilitation of support services. The Advocate works closely with Medical Case Management staff to promote continuity of care and improved health outcomes. Essential Functions: - Complete client intake and assessments in accordance with program guidelines. - Assist in developing and implementing service or care plans. - Provide ongoing psychosocial support and maintain regular contact with assigned clients. - Accompany clients to appointments including medical, housing, court, and financial services as needed. - Prepare clients for interactions with service providers and support them during appointments. - Conduct appointment reminder calls and follow-up outreach. - Facilitate support groups as assigned. - Monitor and track client CD4 and vir...
