Description POSITION: Licensed Professional Counselor/Social Worker: DEPARTMENT : Medical Services STATUS: Exempt REPORTS TO: Director of Clinical Care Position Summary: Serves as a primary care Behavioral Health Consultant team member providing consultation for patients and their primary care providers within the collaborative care model DUTIES AND RESPONSIBILITIES: 15. Ensures delivery of medical care in accordance with the Catholic Moral and Ethical Directives 16. Follows all HIPAA, OSHA, and licensing guidelines, regulations and code of ethics 17. Maintains confidentiality of patient information/records at all times 18. Maintains established San José Clinic policies, procedures, objectives, quality assurance, safety, environmental and infection control 19.
