Overview: Fulltime, benefits eligible role Mon-Fri 8:00am - 5:00pm: Job Description Summary: Assists patients and families with complex medical and behavioral health needs to navigate uneventfully, efficiently, and as effectively as possible through the course of their illness to improve or maintain social, emotional, functional, and physical health. Practices professional social work and promotes patient advocacy in accordance with the NASW Code of Ethics for Social Work. Job Description: Essential Functions: - Functions as a member of an interdisciplinary team and provides care coordination services to patients and their families through screening and identification of patients, outreach to primary care physicians, specialty physicians, and other service providers, completion of the Health Risk Assessment, and assisting with the development of a family-centered plan of care. - Facilitates communication and collaboration amongst the healthcare team.
