The Role: The Targeted Case Manager (TCM) is responsible for supporting the wellness and recovery goals by implementing targeted interventions designed to provide timely, high quality and efficiency care. The TCM is to provide access to services that would encourage clients to resolve problems that interfere with their independence or self-sufficiency as well as maintain themselves in the community.
