Job Summary The Care Manager Assistant supports the coordination of discharge planning and care transitions under the supervision of a Case Manager. This role serves as an interdisciplinary team member and contributes to efficient care delivery by coordinating post-discharge services, managing documentation, and facilitating communication between patients, families, and healthcare providers. The Care Manager Assistant performs responsibilities in a manner that reflects sensitivity to patient needs and supports quality outcomes aligned with hospital goals.
