Case Manager Status: Non-Exempt Reports to: Case Manager Supervisor or Director Designee “Our mission is to provide people with developmental disabilities diverse opportunities to lead fulfilling lives” Qualifications: - Bachelor’s degree in Human Services or a related human services field from an accredited college or university and two years of work experience working directly with persons with disabilities or - Associate’s degree from an accredited college or university and four years of work experience working directly with persons with disabilities; - Must be at least 21 years old; - Valid Pennsylvania Drivers License with acceptable driving record as determined by Motor Vehicle Report and insurance guidelines. Essential Functions: - Responsible for the overall coordination of care, along with some responsibility for the overall and long term elements of the program; - Develops Individual Service Plans with client, family, funding agency representative, and members of treatment team; - Facilitation of Individual Service Plan/Individual Treatment Plan meetings and Interdisciplinary Team Meetings, in cooperation with the Funding agency representative; and meets deadlines for com...
