Job Description Summary: JOB SUMMARY The Solid Organ Transplant Coordinator is responsible for coordination of the evaluation and preparation of patients for organ transplant and long-term management of patients post-transplant. The Transplant Coordinator is responsible for coordinating patient care across the continuum through inpatient and outpatient settings. The Transplant Coordinator should demonstrate independent thinking through a high degree of problem solving and innovation. These skills are required to resolve complex problems that arise unexpectedly and apply appropriate interventions to individual patients while performing daily tasks.
