Application Analyst - Clinical Informatics (Healthy Planet)

Job description

Job Description Reporting to the Manager of EMR Clinical Applications, the Application Analyst (Healthy Planet) will use their clinical knowledge combined with their superior analytical skills to support Mackenzie Health’s EMR (electronic medical record). They will be responsible for the requirements gathering, planning and implementation of new digital health initiatives that integrate Mackenzie Health and the Western York Region Ontario Health Team (WYROHT) community to bring about meaningful transformation to patients, providers and the health system. In providing this leadership, they will work closely with both internal and external partners to build and maintain strong digital health solutions that will enable integrated patient care. The incumbent will be responsible for maintaining strong relationships with the community partners to support and optimize the Healthy Planet Link application for existing pathways and will play an integral role to support the co-design of future integrated care pathways to support chronic disease management.
