Description Job Summary: Enhances the patient experience by providing a seamless navigation process from diagnosis through end of care. Responsible for collaborating with physicians and members of the multidisciplinary teams to triage, coordinate, and consistently manage patient care by assuming the role of resource person for the patient and the multidisciplinary team. Works with patients and families to answer questions, provide emotional support and solve issues concerning the cycle of care. Knowledge, Skills & Abilities: Knowledge of clinical nursing care procedures and protocols. Verbal and written communication skills. Interpersonal skills. Knowledge and understanding of computer systems, tools and programs. Ability to work in a team environment. Responsibilities: Navigates patient through the continuum of care. Educates patients (and possibly family members) on topics such as the nature and status of patient's condition and care deliver goals based on patient specific care plan. Serves as liaison for patients/families, providers, nursing and other appropriate staff.
