Position Summary: At Oak Street, Population Health drives both our clinical and economic model and is focused on delivering high-quality, cost-effective care. As a Sr. Manager, Population Health you will play a critical role in supporting our Oak Street Health patients, providers and business by leading efforts to build a system for supporting patients as they near end-of-life, such as advance care planning and connecting patients to high-quality palliative care and hospice care when needed.
