Opportunity Information Sutter Medical Group is seeking a dedicated and compassionate Skilled Nursing Facility Physician to join our post-acute care team. This position focuses on providing high-quality, patient-centered medical care to patients residing in skilled nursing and rehabilitation facilities. The ideal candidate will have experience managing medically complex, post-acute, and long-term care patients, with a strong commitment to continuity of care, interdisciplinary collaboration, and optimizing patient outcomes during transitions of care.
