Job description

Position Summary: This position is responsible for assisting our Clinical Pharmacist with daily roles by providing personalized care that improves patient outcomes, reduces complications, hospital admissions and ER visits. The Pharmacy technician will specialize in chronic disease states and will be responsible for continued patient management. Support and assist in the coordination of pharmacy services activities including claims utilization analysis, prescription plan design, correspondence and special projects. Essential Duties and Responsibilities include the following: - Assist the pharmacist in the areas of clerical activities while under the Pharmacist’s supervision - Assist pharmacist and physicians in health care insurance for non-covered medications - Conducts medication reconciliation - Scheduling, follow-up and coordinating with patient’s plan of care - Answering telephone calls, obtaining and/or verifying patient information/ medication and health histories - Responding to requests from patients and healthcare providers, answering their questions and referring various inquiries. - Complexity of task/project will vary and requires individuals with a strong aptitude - Do...
