Department: 37527 AMC Grafton - Outpatient Rehabilitation: Mequon Status: Part time Benefits Eligible: Yes Hou rs Per Week: 28 Schedule Details/Additional Information: Certified Hand Therapist. Would need to have experience with making custom splints. Includes one late shift a week. $2,500 sign on bonus. Pay Range: $41.10 - $61.65 Major Responsibilities: Interprets physician referrals, as appropriate, and evaluates the patient's physical, perceptual, cognitive, social, and vocational abilities and their impact on the patient's activities of daily living. Demonstrates advanced evaluation, assessment and treatment skills with complex patients or specific diagnosis or program area. Establishes a plan of care and implements individual treatment based on evaluation results, functional goals and by utilizing appropriate techniques. Provides and/or fabricates assistive devices and/or splints to enhance patient's functional independence. Monitors and evaluates outcomes and appropriately adjusts individualized treatment plan based on patient's response and provides status updates to physicians and other teammates, as necessary for patient care.
