Are you passionate about supporting Aboriginal and Torres Strait Islander Elders to access culturally appropriate aged care services? Gidgee Healing is seeking a dedicated Elder Care Connector to join our Elder Care Support team in Normanton. About Us: Gidgee Healing is an Aboriginal Community Controlled Health Service committed to improving the health and wellbeing of Aboriginal and Torres Strait Islander people across Northwest and Lower Gulf Queensland. We deliver holistic, culturally safe care through community participation and strong care coordination. About the Role: As the Elder Care Connector, you will: - Conduct community outreach to promote aged care services. - Engage with Elders and families to raise awareness of aged care entitlements. - Support clients in identifying their goals and accessing appropriate services. - Assist with My Aged Care registration and review processes. - Advocate for clients and help identify service gaps in the region. - Work closely with the Aged Care Support Coordinator and internal teams.
