Job Purpose & Position Overview The Outpatient Clinical Nurse, LPN is responsible for objectively assessing, planning, implementing, and evaluating nursing care in coordination with a multidisciplinary team to provide “patient centered” care. The Licensed Practical Nurse, under the direction of a provider, will assist in the ongoing management of the patient based on identified psycho-social needs, continuity of care, disease prevention, chronic disease management, and provides patient education to promote Health Literacy.
