The Hospice RN Case Manager plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The Hospice RN Case Manager builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. Position: RN Case Manager for Hospice Salary Range: $88,000-$94,600 annually, depending on experience Service Area: North Central (coverage area: North Loop/North Minneapolis, Fridley, Columbia Heights, Coon Rapids, Blaine, Brooklyn Center, Ham Lake, Andover, East Bethel, Golden Valley, Robbinsdale, Brooklyn Center, Brooklyn Park, Anoka, Coon Rapids, Oak Grove & surrounding). Plus, mileage reimbursement of 72.5 cents per mile.
