Home: Craiglee home Location: Scarborough Position: Social Worker (Full Time, 8am-4pm) KEY RESPONSIBILITIES: - Receive and respond appropriately to inquiries pertaining to Southbridge Roseview which relate to new resident placement, including conducting tours of the home with prospective residents; - To conduct pre-admission procedures including: Screening of applications; Correspondence with applicants to acknowledge receipt of application and to provide information; Pre-admission visits and assessments; Communication with placement agencies; Navigates the resident/family thru the admission process including coordination with the LHIN; - To promote and facilitate a Resident-oriented environment, acting as a Resident/family advocate. Coordinates and is the homes liaison person for Family Council. Leads the Customer Service program within the home; - Coordinate, plan and participate in annual and new admission interdisciplinary team care conferences for residents; - To plan, organize and carry out social work programs/services to meet the ongoing psychosocial needs of the Residents and their families/advocates; - Provides palliative care support to residents and family as needed; -...
