This position is eligible for a $2,500 retention bonus. Job Summary: The Providence Center CCBHC Case Manager ACT I Engagement and Diversion Team supports clients in reducing psychiatric and substance use disorder symptoms, managing stressors, and achieving the highest possible level of independence and community functioning. They deliver person-centered, recovery-oriented case management as part of a multidisciplinary team, providing in-person and community-based support that aligns with program standards. Responsibilities include conducting comprehensive assessments, developing individualized care plans, and connecting clients to housing, healthcare, education, employment, and other community resources. The role also involves ongoing monitoring, crisis intervention, and close collaboration with the client care team and partner agencies to promote recovery while ensuring adherence to all applicable standards and regulations.
