Description St. Elizabeth Nursing Home, an Illuminus Community, is seeking a Life Coach (Certified Social Worker) to join the team. In this role, the individual assist with discharge planning, facilitating adjustment to nursing home placement with residents and their families, and maintaining appropriate records accordingly. This is a salaried position working generally Monday - Friday. No weekends or holidays! Responsibilities: - Maintains and actively promotes effective communication with residents/patients/clients and family members / personal representatives. - Responsible for maintaining a positive image of the entity in the community keeping in alignment with our mission, vision, and values. - Schedules and facilitates quarterly care plan reviews. Provides interdisciplinary staff with appropriate information. Reviews information on the overall plan of care and records appropriate information on the same. Completes MDS as assigned. - Assists in completing a social history on a resident within two weeks of his or her admission.
