Pay Range: $22.89 - 26.32/hr Schedule: Monday - Friday 8am - 4:30pm Primary Office Location: 1400 W Greenleaf Ave., Chicago IL 60626 Secondary Office Location: 8541 S State St., Chicago IL 60619 Job Summary: The Health Mentor is responsible for working with clients enrolled in the InSHAPE program to develop and implement their personal wellness plan. They will provide support, education, and motivation related to physical fitness, nutrition, and wellness to clients in the community. The Health Mentor will model appropriate wellness and provide client-centered, strengths-based, and trauma-informed services to clients with severe mental illnesses and co-occurring substance abuse/addiction disorders utilizing a harm reduction approach. The Health Mentor must have a personal trainer certification or must obtain a personal trainer certification within 6 months of being hired.
