Permanency Case Manager (Canberra)

Job description

The Role As a Permanency Case Manager, you will work closely with team leaders to support children and young people in out-of-home care, along with their siblings, foster carers, birth families and other key supports. Your focus will be on achieving stable, safe and lasting placement outcomes. You will deliver therapeutic case management informed by trauma-aware and evidence-based practices. The role supports the wellbeing of children and young people while working with birth families to strengthen functioning and, where appropriate, support family restoration. Where restoration is not possible, you will assist in establishing alternative permanency options such as adoption, guardianship or long-term foster care. This is a hands-on role involving direct engagement with children, young people, carers and families. You will manage complex cases, provide practical and emotional support, and complete detailed documentation and reporting. Service delivery occurs across homes, offices and community settings, with occasional work outside standard hours to respond to urgent needs.
