In this role, you will build and maintain referral relationships, evaluate patients for eligibility, and coordinate smooth transitions from hospitals, SNFs, ALFs, and physician practices to home-based care. This position requires strong clinical judgment, relationship-building skills, and the ability to thrive in a performance-driven environment.
