Job Summary: The Social Worker-MSW Urgent Care serves as a key member of the patient care delivery team. Identifies patient and family psychosocial and environmental needs related to urgent treatment and discharge. Works collaboratively with other members of the interdisciplinary team in developing and implementing a comprehensive, integrated discharge plan. Makes appropriate referrals for community services; may provide emotional support and counseling services, crisis intervention. Assesses and intervenes in situations involving child or elder abuse/neglect, domestic violence. Provides care and services to all patient groups: Neonatal, infant, pediatric, adolescent, adult and elderly. Performs other duties as needed. Education and Experience: Master's Degree in social work required. Minimum one year of experience in clinical setting required. Previous urgent care experience preferred. Knowledge and Skills: In-depth knowledge of case management practice and methodologies. Substantial knowledge of basic nursing concepts and care of the patient as well as education of the adult learner.
