In this role, you will have the opportunity to support patient care and population health through patient and family self-management support and care coordination activities across the care continuum . You’ll be a valuable member of the SAMG Patient Centered Medical Home (PCMH) helping to achieve the missions and goals of patient centered care while working collaboratively with provider care teams, clinic employees, other health care professionals, and community resources.
