Job Description Assists in identifying and utilizing community resources for patients and their families. Liaisons with various agencies regarding social work issues and maintains all pertinent records. Job Responsibility: *ADA Essential Functions - * Assists in identifying and using community resources for patients and/or their families including visits to patient's homes. - * Assists in establishing liaison and communications with other agencies including visits to agencies. - * Maintains a variety of records and reports relating to social work activity - Performs related duties, as required.
