As Circadia’s home health NP, you’ll see patients first in their homes, act on predictive health signals in real time, and coach care teams to turn data into earlier interventions—reducing preventable hospitalizations. You’ll blend direct patient care with tech-to-bedside integration through hands-on training, live demonstrations, and shadowed assessments within the home health setting.
